HPC Care stands for Homemaker Personal Care, a supportive service designed to help individuals with developmental disabilities live more independently in their homes and communities. HPC services include assistance with daily living tasks such as bathing, grooming, meal preparation, medication reminders, mobility support, and community engagement. This care is tailored to each person’s unique needs and goals, promoting safety, dignity, and improved quality of life.
Skilled Nursing refers to medically necessary care provided by licensed nurses, either at home or in a residential setting. These services include wound care, medication administration, chronic condition management, vital sign monitoring, post-surgical care, and coordination with physicians. Skilled nursing is designed to support recovery, manage complex health needs, and ensure optimal health outcomes under the supervision of a registered or licensed practical nurse.
Our Behavioral Support services are designed to promote safety, independence, and a higher quality of life. We work closely with each individual, their family, and support team to develop person-centered strategies that reduce challenging behaviors and encourage positive choices.

Respite Care provides temporary relief for primary caregivers by offering short-term care and support to individuals with developmental disabilities or medical needs. These services ensure the individual’s safety and well-being while allowing caregivers time to rest, attend to personal needs, or handle other responsibilities. Respite care can be provided in-home or in a community setting and is designed to prevent caregiver burnout while maintaining consistent, compassionate support for the individual receiving care.
Companion care focuses on providing emotional support and social interaction for clients who may feel isolated or lonely. Caregivers offer assistance with light housekeeping, grocery shopping, meal preparation, and accompaniment to appointments or social outings. This service is essential for individuals who live alone or those who need non-medical support to maintain a fulfilling and connected lifestyle. Companion care enhances overall well-being by fostering relationships and encouraging engagement in daily activities.
Companion Care

Safe, reliable transportation for appointments, errands, and social outings. Our NMT service ensures clients reach their destinations comfortably and on time, promoting independence and access to essential services.
Non-Medical Transportation (NMT)

Dignified assistance with hygiene and grooming, including bathing, dressing, and toileting. Our Personal Care service helps clients maintain comfort, cleanliness, and confidence in their daily routines.
